Taking a Good Ocular History
History of Present Illness
- Duration, onset, and timing of symptoms Ophthalmology
- Presence, severity, duration (constant or intermittent), and quality of painHistory of similar presentation
- Ocular symptoms: flashes, floaters, double vision, blurry vision, sudden loss of vision, blind spots, or curtain coming down over vision
- Review of systems
- Past Ocular History:
- Do you wear glasses, distance/reading/both? Contact lenses and proper use (no rewearing, sleeping, swimming, showering in them, etc.)? Last time you saw an eye doctor?
- History of eye surgeries, intraocular injections, eye drops
- History of any other eye conditions: cataracts, glaucoma, macular degeneration, diabetic retinopathy, infections, trauma to eye
- Medical History: diabetes, autoimmune disease, immunodeficiency, malignancy
- Family History: family history of any eye diseases as above
- Social History: tobacco, alcohol, illicit drugs, sexual activity, exposures
